Course Summary

This 2.5-hour intermediate LIVE webinar helps SLPs prevent medical errors through root-cause analysis, error reduction strategies, and patient safety practices. Using case studies across settings, it highlights SLP-specific risks, common missed diagnoses, and the SLP’s role in mitigation. Learners gain practical, patient-centered tools to reduce risk, severity, and frequency of errors.  

This course is designed to meet the mandatory biennial Florida CE requirement for speech-language pathologists in the prevention of medical errors pursuant to FL Statute 456.013(7) and Florida Administrative Code 64B20-2.001(3).

Learning outcomes

After this course, you'll be able to:

  • Identify the prevalence and impact of medical errors in health care systematically as well as for the individual patient.
  • Define medical errors including sentinel events. Identify the underlying principles of root cause analysis (RCA), and describe contributing risk factors for medical errors, including problems in implementing RCA within the healthcare sector. 
  • List the most common reasons why, when, and how medical errors occur. 
  • Develop and apply evidence-based strategies to prevent and mitigate medical errors to decrease risk to and improve outcomes for patients, caregivers, staff, and organizations. 
  • Gain insight into scenarios in which speech-language pathologists might be more likely to contribute to a medical error, and also to mitigate them.

The Presenter

Will Farnham, MS, SLP has been practicing as a speech-language pathologist in medical settings across the continuum of care—including acute care, inpatient rehab, skilled nursing, mobile endoscopy, and outpatient venues—since 2012 for the entirety of his career to date since completing his M.S. at Western Kentucky University. He has served as the principal acute care SLP at Ascension Sacred Heart Emerald Coast Hospital since 2022, and has had the opportunity to serve as an internal subject matter expert for multiple local and national interdisciplinary mission-wide initiatives. Will holds multiple certifications both generally as well as in diagnostic and therapeutic modalities, but believes weaning ourselves from a certification-focused mindset in favor of fostering targeted clinical and critical reasoning is the best path forward for the field. He routinely mentors SLP students, new graduates/clinical fellows, and colleagues. He has appeared as a guest on multiple med SLP-focused podcasts, and occasional dabbles in providing more formal continuing education ventures such as this one. Will is inauspiciously most widely-known as the sole proprietor of the meme page Hard to Swallow across multiple social platforms, where he shares clinically-focused original jokes. When not at work or chronically online, Will enjoys a variety of podcasts, middle-distance running (and staring!), serving as the A/V and streaming producer at his church, and going to weekly pub trivia with his wife and daughter as team Sigma Quizzlers.
