By registering for this course, you acknowledge that Brooke Richardson / MedSLP Consultants / The Hosting Facility cannot guarantee that you will not be exposed to or contract a Communicable Disease, such as COVID-19, at this in-person course. As a participant in the course, you agree to abide by all established and applicable COVID-19-related requirements, advisories, policies, procedures, and protocols of the Hosting Facility and/or Brooke Richardson, as well as the CDC, City and State of the Hosting Facility site, and any other governmental authority for the duration of the event. You understand the risk that you may be exposed to, or infected by, a Communicable Disease by attending the in-person course. You knowingly and freely assume all such risks, both known and unknown, and assume full personal responsibility for your participation. You release Brooke Richardson, MedSLP Consultants, the Hosting Facility, and all associated participants, contractors, and other individuals/organizations from liability should you become ill, directly or indirectly, as a result of your participation in the course.
You may not attend this course if you have tested positive for COVID-19 within 5 days of the course date (longer if dictated by Hosting Facility policies).
